Get Involved in YOUR Diabetes Care!

  • See your health care provider every 3 months.
    • Have A1c, blood pressure and feet checked at every visit
    • Have eyes, lipids and kidney function checked once a year
    • Have your feet tested for loss of feeling at least once a year.
  • Take your medicines daily as directed.
  • Know your numbers (see above). Hitting your targets means that treatment is working and SHOULD BE CONTINUED.
  • Check your blood sugar at home. Learn what the numbers mean and how to USE them to improve control.
  • Learn about the carbohydrates in foods and how they affect your diabetes. This knowledge will give you power and freedom.
  • Become the world's expert on YOUR diabetes!
